These Terms and Conditions ("Terms") govern your use of the J.PEER Health website at jpeerhealth.com ("Website") and the home care services provided by J.PEER Health Pty Ltd ABN 16 683 516 761 ("J.PEER Health", "we", "us", "our").
By accessing our Website or engaging our services, you agree to these Terms. If you do not agree, please do not use our Website or services.
J.PEER Health provides home care services to older Australians in Melbourne's south-east, including but not limited to personal care, domestic assistance, nursing support, social and community support, dementia care, respite care, post-hospital care, and 24/7 overnight care.
Services are provided under a written Service Agreement (care plan) entered into between J.PEER Health and each client (or their authorised representative). The Service Agreement governs the terms of your specific care arrangement and takes precedence over these general Terms in matters of service delivery.
Home care services are available to individuals who:
J.PEER Health reserves the right to conduct an assessment prior to commencing services to ensure we can meet the client's care needs safely and appropriately.
For government-funded clients under the Support at Home program, fees are governed by the Commonwealth Home Support Programme and Support at Home program guidelines, including any applicable client contributions assessed by the Department of Health and Aged Care.
For private clients, fees are as quoted in the Service Agreement. Fees are subject to change with reasonable notice (minimum 14 days written notice). Invoices are issued fortnightly unless otherwise agreed. Payment is due within 14 days of the invoice date.
J.PEER Health does not charge exit fees. There are no penalties for ending services in accordance with the notice provisions in your Service Agreement.
All clients of J.PEER Health are entitled to the rights set out in the Aged Care Act 1997 (Cth) and the aged care quality standards, including:
Clients are responsible for providing accurate information relevant to their care, maintaining a safe environment for carers, and notifying J.PEER Health of changes to their care needs or personal circumstances.
Clients may cancel or reschedule visits with a minimum of 24 hours' notice. Late cancellations (less than 24 hours) or no-shows may be charged at the agreed service rate at J.PEER Health's discretion. J.PEER Health will provide reasonable notice of any changes to scheduled visits on our end.
